Student Health and Counseling services are available through Employee and Student Health which is located on the 1st floor in the Downtown Pavilion, 540 North Duke Street. The hours are Monday – Thursday from 6 am – 7:30 pm and on Fridays from 6 am – 4:30 pm. Employee and Student Health is closed on holidays and weekends. You may drop in or call us at 544-5984 to schedule an appointment.

HEALTH SERVICES
The Health Services Program is designed to focus on holistic views and strives to meet the ever-changing needs of a diverse student population. The goals of the program include promoting wellness and self-care and empowering individuals to be self-directed and well-informed consumers of health care services.
The Health Services Program is provided to enrolled students for routine health maintenance and emergency care for injuries and illness. Students may consult Employee and Student Health for health and counseling services and for referrals or advice on health related matters. Students are responsible for the cost of medical expenses incurred through injury, illness or accidents and should have personal health insurance to cover health costs not covered by the Health Services Program. Students enrolled in a degree or certificate program must have a pre-entrance history and physical completed by a physician, nurse practitioner or physician assistant . Immunizations and blood tests to indicate immunity to communicable illnesses as noted on the form and any required vaccinations must also be completed. Hepatitis B shots are strongly encouraged as one may experience exposure during the course of study. If one does refused to obtain the Hepatitis B immunization a waiver must be signed. Tuberculosis skin testing, urine drug testing for substances of abuse and saliva alcohol testing will be completed during orientation. All students in the Associate of Science in Nursing program must complete an annual physical assessment as required by the State Board. Please access this form from Blackboard under the "Student Services" tab, College Health Center link. This form is to be used for your second or subsequent year.
COUNSELING SERVICES
A student desiring to speak to someone may contact Employee and Student Health Service by calling 544-5984. If the student feels additional counseling is desired it is available through Quest Behavioral Health. Each student is entitled to three free confidential sessions. Appointments can be made by calling 800-364-6352.

DISABILITY SERVICES
Staff of Employee and Student Health serve as ADA coordinators and provide services creating full access and provide appropriate accommodation for all students with disabilities. Employee and Student Health must be contacted to request consideration for specifc accommodations or to address compliance issues with the Americans with Disabilities Act.
